Welcome to the National Center for Medical Home Implementation, a national technical assistance center focused on ensuring all children and youth—particularly those with special health care needs—receive care within, and have access to, a medical home.

A medical home is an approach to providing comprehensive primary care that facilitates partnerships between patients, clinicians, medical staff, and families. A medical home extends beyond the four walls of a clinical practice. It includes specialty care, educational services, family support and more.

This Web site provides tools, resources, state specific information, and promising practices in pediatric medical home implementation.
