The National Center for Medical Home Implementation is working to make that number 100%.

The National Center for Medical Home Implementation (National Center) is a cooperative agreement between the Maternal and Child Health Bureau and the American Academy of Pediatrics. With over 15 years of experience, the National Center is focused on ensuring that all children and youth receive care within, and have access to, a medical home.
