Introducing the National Resource Center for Patient/Family-Centered Medical Home (NRC-PFCMH)!

The American Academy of Pediatrics (AAP or Academy) is one of three organizations funded via a new five-year cooperative agreement from the Maternal and Child Health Bureau of the Health Resources and Services Administration (HRSA) to implement innovative programs and activities to address fragmented systems of care for children and youth with special health care needs (CYSHCN).

Formerly known as the National Center for Medical Home Implementation, this new cooperative agreement will support the AAP to continue developing practical resources and tools for state agencies, public health organizations, pediatric clinicians and others implementing family-centered care.

The NRC-PFCMH will also provide targeted technical assistance, trainings, education, practical tools and resources to assist with implementation of patient/family-centered care.
