State Pediatric Medical Home Activities Current ActivitiesMedical Home Chapter Champions Program on Asthma, Allergy, and AnaphylaxisThis program supports the facilitation and dissemination of best practices, evidence-based guidelines, and resources in asthma, allergy, and anaphylaxis for clinicians throughout the state.Blue Cross Blue Shield of Arizona Patient-Centered Medical Home ProgramThis program improves patient care outcomes in selected chronic diseases, including asthma. The program provides resources to help participating clinicians, including pediatrician, implement components of the medical home model.Past ActivitiesDine' for our Children ProjectFunded by the Health Resources and Services Administration Maternal and Child Health Bureau, this project established parent leadership teams across Arizona to empower families and parents as community advocates for child health. Healthy Tomorrows Partnership for Children ProgramWith funding from the Maternal and Child Health Bureau, these community-based projects work to address health disparities and implement components of the pediatric medical home.
