State Pediatric Medical Home Activities Current ActivitiesMedical Home Chapter Champions Program on Asthma, Allergy, and AnaphylaxisThis program supports the facilitation and dissemination of best practices, evidence-based guidelines, and resources in asthma, allergy, and anaphylaxis for clinicians throughout the state.Advancing the Pediatric Medical Home: Colorado's Accountable Care CollaborativeA new program launched through Colorado Medicaid to connect its beneficiaries to primary care physicians and medical homes. Over 200,000 children are enrolled in this program.Alliance for Innovation on Maternal and Child Health Learning CollaborativeColorado is participating in the Maternal and Child Health Bureau funded Alliance for Innovation on Maternal and Child Health Learning Collaborative. As part of this initiative, the American Academy of Pediatrics (AAP) gathered information related to access to care and coverage issues from patient/family and clinician perspectives in Colorado. In addition, the AAP reviewed current Colorado Early Periodic Screening, Diagnostic, and Treatment requirements to compare the services offered with the services recommended within the Bright Futures Guidelines for Health Supervision.Colorado Medical Home InitiativeA statewide effort to establish and promote patient-centered medical homes throughout Colorado. Partners include the Colorado Chapter of the American Academy of Pediatrics. Colorado Children's Healthcare Access ProgramA non-profit organization that provides preventive care through a medical home model to low-income children enrolled in Medicaid. A case study report published by the Commonwealth Fund examines the outcomes of this organization's work.Healthy Tomorrows Partnership for Children ProgramWith funding from the Maternal and Child Health Bureau, these community-based projects address health disparities and implement components of the pediatric medical home. Current projects in Colorado include the following:Get Healthy! Obesity Treatment and Prevention ProjectState Innovation Model Testing AwardColorado was awarded a Centers for Medicare and Medicaid State Innovation Model Testing Award in December 2014. Through this grant, Colorado is implementing strategies to transform a state's health care delivery system through multi-payer payment reform and additional state initiatives.Past ActivitiesChildren's Health Insurance Program Reauthorization Act Quality Demonstration Grants Colorado was the lead on one of 10 grants funded improve health care quality and delivery systems for children enrolled in Medicaid and the Children's Health Insurance Program (CHIP).
