State Pediatric Medical Home Activities Current ActivitiesMedical Home Chapter Champions Program on Asthma, Allergy, and AnaphylaxisThis program supports the facilitation and dissemination of best practices, evidence-based guidelines, and resources in asthma, allergy, and anaphylaxis for clinicians throughout the state.Hawaii Medical Services Association Patient-Centered Medical Home ProgramThis program partners with the Hawaii Chapter of the American Academy of Pediatrics to provide enhanced payment to participating pediatricians. Hawaii Dyson InitiativeThe University of Hawaii Integrated Pediatric Residency Program provides electronic training modules to pediatric residents in community-based medical home implementation.Healthy Tomorrows Partnership for Children ProgramWith funding from the Maternal and Child Health Bureau, these community-based projects address health disparities and implement components of the pediatric medical home.
