  • Illinois Chapter of the American Academy of Pediatrics Releases Annual Medical Home Newsletter 
    The Building Community-Based Medical Homes for Children Illinois newsletter is now available on the Illinois Chapter of the American Academy of Pediatrics (ICAAP) Web site. ICAAP
    has been working on several medical home initiatives over the past three years.  A few have been completed and results are summarized in this seventh edition.  Many other activities continue to support pediatricians, providers, and patients and families around the state.  For more information, please contact Kathy Sanabria at ksanabria@illinoisaap.com.

Current Activities

    • Medical Home Chapter Champions Program on Asthma, Allergy, and Anaphylaxis
      This program supports the facilitation and dissemination of best practices, evidence-based guidelines, and resources in asthma, allergy, and anaphylaxis for clinicians throughout the state.
    • Medical Home Network: Building Partnership for Better Health
      A virtual network of clinicians that work to provide care aligned with the medical home model. Participating clinicians work in multiple healthcare settings throughout Chicago including Federally Qualified Health Centers and La Rabida Children's Hospital.
    • Illinois Health Connect
      Serving as Illinois' primary care case management program, this program services populations covered by Medicaid and the Child Health Insurance Program. Participating primary care clinicians provide medical home services to children and youth covered by these public insurance programs.
    • Care Coordination Innovations Project
      Illinois is implementing two innovative health care delivery models that will provide comprehensive care coordination services to Medicaid beneficiaries.
    • Healthy Tomorrows Partnership for Children Program
      With funding from the Maternal and Child Health Bureau, these community-based projects address health disparities and implement components of the pediatric medical home. Current projects in Illinois include the following:

      The Family-to-Family Health Information Center and Family Voices State Affiliate Organization of Illinois provides resources, peer support, education, and information to families of children with special health care needs.

      A profile of Illinois' state Title V program, includes contact information for the Maternal and Child Health and the Children and Youth with Special Health Care Needs Program.
    • Illinois Medicaid
      State Medicaid Directors Contact Information
      Find information on Medicaid programs, resources, providers, and contacts within Illinois.
    • Voices for Illinois Children
      This non-profit organization works with families, clinicians, communities, and policy makers to ensure a healthy environmental for all children in Illinois.
