ICAAP has been working on several medical home initiatives over the past three years. A few have been completed and results are summarized in this seventh edition. State Pediatric Medical Home Activities Current ActivitiesMedical Home Chapter Champions Program on Asthma, Allergy, and AnaphylaxisThis program supports the facilitation and dissemination of best practices, evidence-based guidelines, and resources in asthma, allergy, and anaphylaxis for clinicians throughout the state.Medical Home Network: Building Partnership for Better HealthA virtual network of clinicians that work to provide care aligned with the medical home model. Participating clinicians work in multiple healthcare settings throughout Chicago including Federally Qualified Health Centers and La Rabida Children's Hospital.Illinois Health ConnectServing as Illinois' primary care case management program, this program services populations covered by Medicaid and the Child Health Insurance Program. Participating primary care clinicians provide medical home services to children and youth covered by these public insurance programs.Care Coordination Innovations ProjectIllinois is implementing two innovative health care delivery models that will provide comprehensive care coordination services to Medicaid beneficiaries.
