 State Pediatric Medical Home Activities

Current

  • Alliance for Innovation on Maternal and Child Health Learning Collaborative
    Kentucky is participating in the Maternal and Child Health Bureau funded Alliance for Innovation on Maternal and Child Health Learning Collaborative. As part of this initiative, the American Academy of Pediatrics (AAP) gathered information related to access to care and coverage issues from patient/family and clinician perspectives in Kentucky. In addition, the AAP reviewed current Kentucky Early Periodic Screening, Diagnostic, and Treatment requirements to compare the services offered with the services recommended within the Bright Futures Guidelines for Health Supervision.

  • Healthy Tomorrows Partnership for Children Program
    With funding from the Maternal and Child Health Bureau, these community-based projects address health disparities and implement components of the pediatric medical home. Current projects in Kentucky include the following:
