State Pediatric Medical Home Activities Current ActivitiesMedical Home Chapter Champions Program on Asthma, Allergy, and AnaphylaxisThis program supports the facilitation and dissemination of best practices, evidence-based guidelines, and resources in asthma, allergy, and anaphylaxis for clinicians throughout the state.Primary Care Access and Stabilization GrantThis grant works with multiple health care settings, including Federally Qualified Health Centers and primary care practices, to implement coordinated, culturally competent, family-centered care.Bayou Health NetworksA Medicaid Managed Care program which establishes networks to improve access to and quality of primary care services, with specific provisions for the pediatric population. Louisiana Health Care Quality ForumA volunteer based organization that promotes health information technology, quality measurement, and medical home. The organization published a Medical Home Implementation Resource Tool Kit.
