 State Pediatric Medical Home Activities

Current Activities

  • Maine Patient-Centered Medical Home Pilot
    This pilot project supports pediatric practices as they work to transform to patient-centered medical homes through learning collaboratives, practice coaches, and consultation with key experts.
  • MaineCare Accountable Communities Initiative
    This Medicaid program strengthens primary care and improves transitions of care for Medicaid beneficiaries in Maine, including children and youth with special health care needs.
  • Maine Health Home State Plan Amendment
    Working closely with the Maine Patient-Centered Medical Home Pilot, this health home program provides comprehensive, continuous, coordinated care to Medicaid beneficiaries with chronic conditions.
  • Maine Child Health improvement Partnership
    This project works to optimize the health of all children in Maine through public/private partnerships that measure quality improvement.
  • Developmental Systems Integration Project
    This project focuses on cross-sector integration and care coordination to ensure the health and development of all children and youth in Maine. Partners include the Maine Chapter of the American Academy of Pediatrics.
  • State Innovation Model Testing Award
    Maine was awarded a Centers for Medicare and Medicaid State Innovation Model Testing Award in February 2013. Through this grant, Maine is implementing strategies to transform a state's health care delivery system through multi-payer payment reform and additional state initiatives.
