State Pediatric Medical Home Activities Current ActivitiesMaineCare Accountable Communities InitiativeThis Medicaid program strengthens primary care and improves transitions of care for Medicaid beneficiaries in Maine, including children and youth with special health care needs.Maine Health Home State Plan AmendmentWorking closely with the Maine Patient-Centered Medical Home Pilot, this health home program provides comprehensive, continuous, coordinated care to Medicaid beneficiaries with chronic conditions. Maine Child Health improvement PartnershipThis project works to optimize the health of all children in Maine through public/private partnerships that measure quality improvement. Developmental Systems Integration ProjectThis project focuses on cross-sector integration and care coordination to ensure the health and development of all children and youth in Maine. Partners include the Maine Chapter of the American Academy of Pediatrics. State Innovation Model Testing AwardMaine was awarded a Centers for Medicare and Medicaid State Innovation Model Testing Award in February 2013.
