State Pediatric Medical Home Activities Current ActivitiesMedical Home Chapter Champions Program on Asthma, Allergy, and AnaphylaxisThis program supports the facilitation and dissemination of best practices, evidence-based guidelines, and resources in asthma, allergy, and anaphylaxis for clinicians throughout the state.Care First Patient-Centered Medical Home ProgramA program of Blue Cross Blue Shield of Maryland, this region-wide patient-centered medical home program works to lower cost of care and improve quality.Healthy Tomorrows Partnership for Children ProgramWith funding from the Maternal and Child Health Bureau, these community-based projects address health disparities and implement components of the pediatric medical home. Current projects in Maryland include the following:The Baltimore Citywide Child Protection Team: Improving Access to Care for Children with Suspected Abuse and NeglectPast ActivitiesMaryland Multi-Payer Patient-Centered Medical Home ProgramA 3-year pilot program that included 53 primary care practices and at least 5 pediatric practices. Pediatric specific quality measures were included in this project, as well as financial incentives for patient-centered medical home implementation.Maryland Community of Care Consortium for Children with Special Health Care NeedsA multi-stakeholder initiative working to improve the systems of care for children and youth with special health care needs.
