State Pediatric Medical Home Activities Current ActivitiesMedical Home Chapter Champions Program on Asthma, Allergy, and AnaphylaxisThis program supports the facilitation and dissemination of best practices, evidence-based guidelines, and resources in asthma, allergy, and anaphylaxis for clinicians throughout the state.Blue Cross Blue Shield of Michigan Patient-Centered Medical Home ProgramThe largest private insurer program in the country focused on payments for patient-centered medical home implementation includes over 4,000 primary care physicians, including pediatric and adolescent medicine physicians. Health Net of West MichiganA non-profit organization that assists adults and children with navigating the healthcare system. This program serves pediatric Medicaid beneficiaries. Michigan Health Improvement AllianceA regional collaborative working to provide patient-centered, coordinated, health care services to adults and children in central Michigan.Advancing the Pediatric Medical Home: Michigan Primary Care Transformation ProjectOver 50 pediatric practices participate in this initiative to achieve medical home recognition and receive enhanced payment for care coordination, practice transformation, and achievement of specific performance measures.Michigan Primary Care ConsortiumA membership organization that implements patient-centered medical homes throughout the state of Michigan.State Innovation Model Testing AwardMichigan was awarded a Centers for Medicare and Medicaid State Innovation Model Testing Award in December 2014.
