Past Activities

  • Center for Medical Home Improvement
    An organization affiliated with the Crotched Mountain Foundation that provided tools, resources, and facilitated pilot projects for pediatric medical home implementation.
  • New Hampshire Citizen's Health Initiative Reimbursement and Medical Home Work Group
    A joint workgroup between private and public payers that promoted creation of quality measures and benchmarks for primary, specialty, and ambulatory care in the context of medical home.
  • The Medical Home Project for Children with Special Health Care Needs
    This project partnered with state Title V agencies and the Center for Medical Home Improvement to develop a pediatric network and registry, create state policy to support medical home transformation, and provide technical assistance to pediatric and family medicine practices as they transformed into medical homes.
  • Healthy Tomorrows Partnership for Children Program Grantees
    With funding from the Maternal and Child Health Bureau, these community-based projects work to address health disparities and implement components of the pediatric medical home. Past projects in New Hampshire include the following:
