    The Newark School-Based Health Center Program is a Healthy Tomorrows Partnership for Children Program (HTPCP) located in Newark, New Jersey. The program's leadership shared the program's strategies and success on the implementation of team-based care with the National Center for Medical  Home Implementation (National Center).
  • Interview with the Children's Medical Services and Advocacy Center
    The Children's Medical Services and Advocacy Center is a Healthy Tomorrows Partnership for Children Program (HTPCP) in New Jersey. The program's leadership shared its strategies and successes in the integration of behavioral health and primary care for vulnerable children and youth with the National Center for Medical Home Implementation (National Center).

      • Healthy Tomorrows Partnership for Children Program
        With funding from the Maternal and Child Health Bureau, these community-based projects  address health disparities and implement components of the pediatric medical home. Past projects in New Jersey include the following:

  • Statewide Parent Advocacy Network
    The Family Health Information Center and Family Voices State Affiliate Organization of New Jersey provides education, information, and peer support to families of children with special health care needs. Resources are available on this Web site in multiple languages.
  • New Jersey Title V
    A profile of New Jersey's state Title V program, includes contact information for the Maternal and Child Health and the Children and Youth with Special Health Care Needs Programs.

    The Child Health Insurance Program of New Jersey; includes resources and information on how to enroll.
  • MOM2MOM Helpline Program
    A 24 hours, 7 day per week helpline for parents in Essex and Union Counties of New Jersey for peer support and counseling as well as connection to community resources for children and youth with special health care needs.
