 State Pediatric Medical Home Activities

Current Activities

  • Alliance for Innovation on Maternal and Child Health Learning Collaborative
    Rhode Island is participating in the Maternal and Child Health Bureau funded Alliance for Innovation on Maternal and Child Health Learning Collaborative. As part of this initiative, the American Academy of Pediatrics (AAP) gathered information related to access to care and coverage issues from patient/family and clinician perspectives in Rhode Island. In addition, the AAP reviewed current Rhode Island Early Periodic Screening, Diagnostic, and Treatment requirements to compare the services offered with the services recommended within the Bright Futures Guidelines for Health Supervision.

  • Pediatric Practice Enhancement Project
    This program trains parents to serve as peer navigators within pediatric practices to assist with cultural competency, care coordination, and family-centered care.
  • Rhode Island Chronic Care Transformation Collaborative
    A multi-payer initiative that assists primary care practices to transform to medical homes. Nearly all of Rhode Island payers participate in this project.
      • Patient-Centered Medical Home Kids Project
        In partnership with the Rhode Island Chapter of the American Academy of Pediatrics, this new program is assisting ten pediatric primary care practices in Rhode Island to transform to medical homes and provide enhanced payment.
  • Rhode Island Health Home Initiative
    Rhode Island provides comprehensive primary care services within a medical home model to pediatric Medicaid beneficiaries through its CEDARR Family Center Health Home program.
  • State Innovation Model Testing Award
    Rhode Island was awarded a Centers for Medicare and Medicaid State Innovation Model Testing Award in December 2014. Through this grant, Rhode Island is implementing strategies to transform a state's health care delivery system through multi-payer payment reform and additional state initiatives.
