 State Pediatric Medical Home Activities

Current Activities

  • South Carolina Medical Home Network Program
    A primary care case management program for Medicaid beneficiaries in South Carolina provides coordinated, comprehensive care within a medical home model. Participating practices are required to report on multiple quality indicators including those related to childhood immunizations, among others. All children under foster care are being moved into this medical home network program.
  • Quality through Technology and Innovation in Pediatrics
    Originally started as part of the Children's Health Insurance Program Reauthorization Act Quality Demonstration Grant (see below), this program has continued beyond this project due to Medicaid funding. The project improves children's quality of care, develop team-based care within medical homes, and address mental health challenges of children throughout Couth Carolina.

    The Family Health Information Center and Family Voices State Affiliate Organization of South Carolina provides education, information, and peer support to families of children with special health care needs.
