Vermont Child Health Improvement ProgramA program focusing on improving pediatric population health through quality improvement initiatives and cross-system collaboration. A Learning Community of Improvement Partnership in State CYSHCN ProgramsVermont provides leadership and technical assistance to four state-based improvement partnerships amount Children and Youth with Special Health Care Needs (CYSHCN) programs funded through Title V.Child Health Advances Measured in PracticeThis multi-year improvement project will focus on improving the health outcomes of pediatric Medicaid beneficiaries, as well as assisting the clinicians and professionals that care for them.Vermont Program for Evidence in PracticeThis project improves the treatment and outcomes of children with emotional and behavioral health disorders within the context of the medical home.State Innovation Model Testing AwardVermont was awarded a Centers for Medicare and Medicaid State Innovation Model Testing Award in February 2013. Through this grant, Vermont is implementing strategies to transform a state's health care delivery system through multi-payer payment reform and additional state initiatives.Past Activities Pediatric Care Coordination Learning CollaborativeThrough funding from the Maternal and Child Health Bureau, Vermont enhanced care coordination through two practice-based learning collaboratives.Children's Health Insurance Program Reauthorization Act Quality Demonstration GrantsVermont was a partner on one of 10 grants funded to improve health care quality and delivery systems for children enrolled in Medicaid and the Children's Health Insurance Program (CHIP). Improving Health Outcomes for Children ProgramFormed as a result of the Children's Health Insurance Program Reauthorization Act Quality Demonstration Grants, this project assisted practices in becoming formally recognized as medical homes. Healthy Tomorrows Partnership for Children ProgramWith funding from the Maternal and Child Health Bureau, these community-based projects address health disparities and implement components of the pediatric medical home.
