Past Activities

  •  Pediatric Care Coordination Learning Collaborative
    Through funding from the Maternal and Child Health Bureau, Vermont enhanced care coordination through two practice-based learning collaboratives.
  • Children's Health Insurance Program Reauthorization Act Quality Demonstration Grants
    Vermont was a partner on one of 10 grants funded to improve health care quality and delivery systems for children enrolled in Medicaid and the Children's Health Insurance Program (CHIP).
  • Improving Health Outcomes for Children Program
    Formed as a result of the Children's Health Insurance Program Reauthorization Act Quality Demonstration Grants, this project assisted practices in becoming formally recognized as medical homes.
  • Healthy Tomorrows Partnership for Children Program
    With funding from the Maternal and Child Health Bureau, these community-based projects address health disparities and implement components of the pediatric medical home. Past projects in Vermont include the following:
