 State Pediatric Medical Home Activities

Current Activities

  • Medical Home Chapter Champions Program on Asthma, Allergy, and Anaphylaxis
    This program supports the facilitation and dissemination of best practices, evidence-based guidelines, and resources in asthma, allergy, and anaphylaxis for clinicians throughout the state.
  • Medallian 3.0 Managed Care Contract
    This contract requires Virginia Managed Care Organizations to implement at least one Medallion Care System Partnership (MCSP) to identify, monitor, and assign members with complex or chronic health conditions to a Health Care Home. All MCSPs are required to target pediatric populations.
  • Healthy Tomorrows Partnership for Children Program
    With funding from the Maternal and Child Health Bureau, these community-based projects address health disparities and implement components of the pediatric medical home. Current projects in Virginia include the following:

Past Activities

  • Cigna and Holston Medical Group Patient-Centered Medical Home Pilot
    A regional pilot program in southwest Virginia focusing on children and adults with complex conditions. The program implemented electronic health records, extended hours and accessibility to practices, and enhanced payment to clinicians for providing care within the medical home model.
  • Healthy Tomorrows Partnership for Children Program
    With funding from the Maternal and Child Health Bureau, these community-based projects address health disparities and implement components of the pediatric medical home. Past projects in Virginia include the following:
