 State Pediatric Medical Home Activities

Current Activities

  • Medical Home Chapter Champions Program on Asthma, Allergy, and Anaphylaxis
    This program supports the facilitation and dissemination of best practices, evidence-based guidelines, and resources in asthma, allergy, and anaphylaxis for clinicians throughout the state.
  • Wisconsin Statewide Medical Home Initiative
    In partnership with the Wisconsin chapter of the American Academy of Pediatrics, this program provides support, technical assistance, and resources to clinicians, practices, and families throughout Wisconsin to assist with medical home implementation.
  • Great Lakes Inter-Tribal Council Children and Youth with Special Health Care Needs Project
    This program provides support, information, and care coordination services for Native American families with children and youth with special health care needs. Special emphasis is placed on enhancing access to primary care services for this population.
  • Medical Home Systems Integration Project
    The goal of this Maternal and Child Health funded initiative is to increase by 20% number of children and youth with special health care needs who receive a medical home in Wisconsin by 2017.
  • Care4Kids
    A Medicaid program that provides comprehensive primary care to children in out-of-home care. Emphasis is placed on providing care within the core tenets of the medical home.

  • Healthy Tomorrows Partnership for Children Program
    With funding from the Maternal and Child Health Bureau, these community-based projects address health disparities and implement components of the pediatric medical home. Past projects in Wisconsin include the following:
