State Pediatric Medical Home Activities Current ActivitiesMedical Home Chapter Champions Program on Asthma, Allergy, and AnaphylaxisThis program supports the facilitation and dissemination of best practices, evidence-based guidelines, and resources in asthma, allergy, and anaphylaxis for clinicians throughout the state. Wyoming currently has a vacancy for a medical home chapter champion. Alliance for Innovation on Maternal and Child Health Learning CollaborativeWyoming is participating in the Maternal and Child Health Bureau funded Alliance for Innovation on Maternal and Child Health Learning Collaborative. As part of this initiative, the American Academy of Pediatrics (AAP) gathered information related to access to care and coverage issues from patient/family and clinician perspectives in Wyoming. In addition, the AAP reviewed current Wyoming Early Periodic Screening, Diagnostic, and Treatment requirements to compare the services offered with the services recommended within the Bright Futures Guidelines for Health Supervision.Wyoming Patient Centered Medical HomeWyoming's Department of Health provides enhanced payments to participating primary care practices based on a patient-centered medical home attestation process. Participating practices do not need to achieve formal medical home recognition or certification to complete the attestation process. Participating providers report on nine clinical quality measures including those specific to pediatrics.
