    • Interview with Clinic in the Park
      Clinic in the Park is a Healthy Tomorrows Partnership for Children Program located in Orange County, California. The program's medical director shared details about how the program collaborates with community and state partners to promote access to pediatric medical homes for children and youth.

Current Activities

    • Medical Home Chapter Champions Program on Asthma, Allergy, and Anaphylaxis
      This program supports the facilitation and dissemination of best practices best practices, evidence-based guidelines, and resources in asthma, allergy, and anaphylaxis for clinicians throughout the state.
    • Alliance for Innovation on Maternal and Child Health Learning Collaborative
      California is participating in the Maternal and Child Health Bureau funded Alliance for Innovation on Maternal and Child Health Learning Collaborative. As part of this initiative, the American Academy of Pediatrics (AAP) gathered information related to access to care and coverage issues from patient/family and clinician perspectives in California. In addition, the AAP reviewed current California Early Periodic Screening, Diagnostic, and Treatment requirements to compare the services offered with the services recommended within the Bright Futures Guidelines for Health Supervision.

    • California Community Care Coordination Collaborative
      This learning collaborative brings together community coalitions with representatives from early childhood development programs, primary health care clinics, and public health in order to enhance care coordination for children and youth with special health care needs.
    • California Children's Services Demonstration Project
      Part of California's "Bridge to Reform" Medicaid Demonstration waiver, this project tests new approaches to improve quality of care for children with special health care needs consistent with the medical home model.
    • SmartCare Behavioral Consultation Services
      The California Chapter 3 of the American Academy of Pediatrics is a partner on this project that enhances care coordination between primary care and behavioral health services within pediatric medical homes.
