State Pediatric Medical Home Activities Current ActivitiesMedical Home Chapter Champions Program on Asthma, Allergy, and AnaphylaxisThis program supports the facilitation and dissemination of best practices best practices, evidence-based guidelines, and resources in asthma, allergy, and anaphylaxis for clinicians throughout the state.Alliance for Innovation on Maternal and Child Health Learning CollaborativeCalifornia is participating in the Maternal and Child Health Bureau funded Alliance for Innovation on Maternal and Child Health Learning Collaborative. As part of this initiative, the American Academy of Pediatrics (AAP) gathered information related to access to care and coverage issues from patient/family and clinician perspectives in California. In addition, the AAP reviewed current California Early Periodic Screening, Diagnostic, and Treatment requirements to compare the services offered with the services recommended within the Bright Futures Guidelines for Health Supervision.California Community Care Coordination CollaborativeThis learning collaborative brings together community coalitions with representatives from early childhood development programs, primary health care clinics, and public health in order to enhance care coordination for children and youth with special health care needs.California Children's Services Demonstration ProjectPart of California's "Bridge to Reform" Medicaid Demonstration waiver, this project tests new approaches to improve quality of care for children with special health care needs consistent with the medical home model.
