State Pediatric Medical Home Activities Current ActivitiesIdaho Healthcare CoalitionIdaho created the Idaho Healthcare Coalition (IHC) to build off the work of the Idaho Medical Home Collaborative, which ended in December 2014. The IHC will continue to advance the medical home model for children and adults and lead the State Healthcare Innovation Plan. State Innovation Model Testing AwardIdaho was awarded a Centers for Medicare and Medicaid State Innovation Model Testing Award in December 2014. Through this grant, Idaho is implementing strategies to transform a state's health care delivery system through multi-payer payment reform and additional state initiatives.Past ActivitiesChildren's Health Insurance Program Reauthorization Act Quality Demonstration Grants Idaho was a partner on one of 10 grants funded to improve health care quality and delivery systems for children enrolled in Medicaid and the Children's Health Insurance Program (CHIP).Idaho Medical Home ProjectLaunched by the Idaho Medical Home Collaborative, this 2-year multi-payer pilot project focused on implementation of medical home among multiple clinics in Idaho, including pediatric and family medicine clinics.
