 State Pediatric Medical Home Activities

Current Activities

    • Idaho Healthcare Coalition
      Idaho created the Idaho Healthcare Coalition (IHC) to build off the work of the Idaho Medical Home Collaborative, which ended in December 2014. The IHC will continue to advance the medical home model for children and adults and lead the State Healthcare Innovation Plan.
