Past Activities

  • Michigan Children's Health Access Program
    A demonstration project which established regional medical homes for pediatric Medicaid beneficiaries.
  • Michigan State University Primary Care Research and Evaluation Program
    A 3-year grant funded study examining the effectiveness of two patient-centered medical home pilot studies and outcomes for patients, clinicians, and staff.
  • Healthy Tomorrows Partnership for Children Program
    With funding from the Maternal and Child Health Bureau, these community-based projects address health disparities and implement components of the pediatric medical home. Past projects in Michigan include the following:
