  • Mississippi Integrated Community Systems Overview
    With funding from the Maternal and Child Health Bureau, Mississippi created a system of services for children and youth with special health care needs that focus on medical home development, early and continuous screening, and youth transition.
  • Healthy Tomorrows Partnership for Children Program
    With funding from the Maternal and Child Health Bureau, these community-based projects address health disparities and implement components of the pediatric medical home. Past projects in Mississippi include the following:
