 State Pediatric Medical Home Activities

Current Activities

  • Alliance for Innovation on Maternal and Child Health Learning Collaborative
    Montana is participating in the Maternal and Child Health Bureau funded Alliance for Innovation on Maternal and Child Health Learning Collaborative. As part of this initiative, the American Academy of Pediatrics (AAP) gathered information related to access to care and coverage issues from patient/family and clinician perspectives in Montana. In addition, the AAP reviewed current Montana Early Periodic Screening, Diagnostic, and Treatment requirements to compare the services offered with the services recommended within the Bright Futures Guidelines for Health Supervision.

  • Passport to Health
    The Montana primary care case management program for children and youth covered through Medicaid. The program emphasizes family and patient centeredness, team-based care, and care coordination.
  • Multi-payer Medical Home Program
    A voluntary statewide multi-payer patient-centered medical home program that includes multiple pediatric practices.
