Current Activities

  • Nebraska Multi-payer Patient-Centered Medical Home Pilot
    This 2-year pilot project includes a full list of pediatric measures such as immunization, asthma, and developmental screening.
  • Healthy Tomorrows Partnership for Children Program
    With funding from the Maternal and Child Health Bureau, these community-based projects address health disparities and implement components of the pediatric medical home. Current projects in Nebraska include the following:


  • Nebraska Medicaid Medical Home Pilot Program
    Implemented in medically underserved rural communities, this pilot program aligned payments for implementation of patient- and family- centered medical homes, specifically for Medicaid populations.
