 State Pediatric Medical Home Activities

Current Activities

  • SoonerCare Choice
    This primary care case management program provides Oklahoma's Medicaid beneficiaries with health care within the medical home model. The program includes the implementation of a pay-per-performance program for participating clinicians.
  • Fostering Hope Clinic
    In partnership with the University of Oklahoma Health Sciences Center, this project provides a medical home for children in foster care in Oklahoma County.

 Past Activities

  • Healthy Tomorrows Partnership for Children Program
    With funding from the Maternal and Child Health Bureau, these community-based projects address health disparities and implement components of the pediatric medical home. Past projects in Oklahoma include the following:
